Located south of the Tule River and west of Lake Success, Gateway Post Acute is proud to serve the Porterville community and surrounding areas. Our experienced team provides 24-hour skilled nursing, short-term rehabilitation, long-term care, respiratory services, IV antibiotic therapy, advanced wound vac treatment, hospice support, and complex wound care, allowing residents to receive a wide range of services in one convenient location.
The Gateway Post Acute Team
Our skilled nurses, therapists, caregivers, and support staff work together to provide high-quality, resident-centered care. Every member of our team shares a commitment to supporting recovery, promoting independence, and enhancing quality of life.
Leadership Team
Our Licensed Nursing Home Administrator (LNHA) oversees facility operations, staffing, services, and programs. Working alongside the Administrator are the Medical Director, Director of Nursing, Rehabilitation Director, Social Services Director, and Environmental Services Director, who help ensure residents receive exceptional care and support.
Nursing / Clinical Staff
Led by our Director of Nursing, our nursing team provides compassionate, around-the-clock clinical care. Registered Nurses (RNs) oversee assessments, treatment plans, and care coordination, while Certified Nursing Assistants (CNAs) assist residents with daily activities and personal care needs.
Therapy / Rehabilitation
Our rehabilitation professionals develop individualized therapy plans designed to help residents regain strength, mobility, and independence. Physical, Occupational, and Speech Therapists work closely with residents to support recovery and maximize functional outcomes.
Social Services Staff
Our Social Services team helps residents and families navigate the emotional, social, and practical aspects of care. They provide support, advocacy, discharge planning assistance, and access to valuable community resources.
Registered Dietitians
Our Registered Dietitians oversee nutritional services and meal planning to help ensure residents receive balanced meals that support their health, wellness, and individual dietary needs.
Environmental Services
The Environmental Services team helps maintain a clean, safe, and comfortable environment for residents, visitors, and staff. Their responsibilities include housekeeping, laundry services, facility maintenance, and infection prevention practices.
